While transforming into a ghoul in Fallout 76 will obviously alter the character's appearance, the change itself will be far from superficial. That's according to the game's creative director, Jonathan Rush, who said as much during a recent Gamescom interview with TheGamer. One of the ways in which playing as a ghoul will change one's experience of exploring Appalachia will come in the form of unique NPC interactions, as some characters will treat these heavily irradiated creatures differently from humans. However, this change won't apply retroactively, with ghoul-specific interactions being exclusive to future content.

It also sounds like ghouls will get some unique ways to progress and complete Fallout 76 quests. The full extent of the changes brought about by playing as a ghoul is currently unclear, as the creative director did not provide any additional details on the matter.

In the meantime, what's certain is that ghoul players will also be able to transform back into humans with minimal effort. Although this ability is not exactly lore-friendly, it's meant to encourage Fallout 76 players to give ghouling a go by not requiring them to sacrifice their character if they end up not liking the experience or grow tired of it. The ability to transform into a ghoul will be tied to a quest that's unlocked at level 50.
How much Bethesda ends up building on this feature moving forward remains to be seen. Its executive producer, Todd Howard, recently revealed that the developer already has years' worth of Fallout 76 content planned, suggesting the live-service RPG will keep getting supported until at least 2026. The intensity of the developer's efforts to create unique content for ghoul players from here on out will likely depend on how popular this feature ends up being. Should enough people respond positively to it, ghouls might also end up being playable in Fallout 5, as Rush recently revealed.
